FASCINA $1,000 Giveaway: Official Rules

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID IN FLORIDA, NEW YORK AND RHODE ISLAND AND WHERE PROHIBITED OR RESTRICTED BY LAW.

1. Sponsor

Gal Yaacobi Holdings LLC (dba FASCINA), 30 N Gould St, Ste N, Sheridan, WY 82801, USA ("Sponsor"), operator of fascinahealth.com, runs the FASCINA $1,000 Giveaway (the "Sweepstakes"). Contact: tryfascina@gmail.com.

2. Eligibility

Open only to legal residents of the fifty (50) United States and the District of Columbia, excluding residents of Florida, New York and Rhode Island, who are eighteen (18) years of age or older (nineteen (19) in Alabama and Nebraska, twenty-one (21) in Mississippi) at the time of entry. Employees, officers and directors of Sponsor, its suppliers, fulfillment partners and advertising agencies, and the immediate family members (spouse, parents, siblings, children) and household members of each, are not eligible. Void in Florida, New York and Rhode Island and where prohibited.

3. Entry Period

The Sweepstakes begins at 12:00:01 a.m. Eastern Time ("ET") on October 25, 2026 and ends at 11:59:59 p.m. ET on October 31, 2029 (the "Entry Period"). Sponsor's computer is the official time-keeping device.

4. How to Enter

There are two ways to enter. Every entry, whichever way it is made, has the same chance of winning.

(a) With a qualifying order. Place a new subscription order for Ultimate Fascia Support™ on fascinahealth.com during the Entry Period and you will automatically receive one (1) entry per qualifying order. An order that is cancelled, refunded or charged back before the drawing does not qualify, and its entry is void.

(b) Free method of entry (no purchase). Visit fascinahealth.com/pages/sweepstakes-entry during the Entry Period and complete the free entry form with your first and last name, email address, mailing address and date of birth, and agree to these Official Rules. Limit one (1) free entry per person per day. Or hand-print the same information on a 3" x 5" card and mail it in a stamped envelope to FASCINA $1,000 Giveaway, Gal Yaacobi Holdings LLC (dba FASCINA), 30 N Gould St, Ste N, Sheridan, WY 82801, USA, postmarked by October 31, 2029 and received by November 7, 2029. Limit one (1) card per envelope. Each valid mail-in card earns one (1) entry, the same as one qualifying order. Entries from residents of Florida, New York and Rhode Island are void, whichever way they are made.

Entries made by script, macro, bot or any other automated means, or with multiple or fictitious email addresses, are void. Sponsor is not responsible for lost, late, incomplete, damaged or misdirected entries.

5. Drawing and Odds

On or before November 14, 2029 (within fourteen (14) days after the Entry Period ends), Sponsor will select one (1) potential winner in a random drawing from all eligible entries received during the Entry Period. Sponsor administers the Sweepstakes and will conduct the drawing using a random number generator, and will keep a record of the drawing. Sponsor's decisions are final. Odds of winning depend on the total number of eligible entries received.

6. Prize

One (1) Grand Prize: one thousand U.S. dollars ($1,000.00), paid to the verified winner by check or ACH bank transfer, as determined by Sponsor, within thirty (30) days after the winner's eligibility is verified and the required forms are received. Approximate Retail Value ("ARV"): $1,000.00. Total ARV of all prizes: $1,000.00. The prize is non-transferable and no substitution is allowed. The winner is solely responsible for all federal, state and local taxes on the prize, and will be required to provide a completed IRS Form W-9 and will receive an IRS Form 1099 for the prize value.

7. Winner Notification and Verification

The potential winner will be notified by email within five (5) business days after the drawing and must reply within seven (7) days. The potential winner may be required to sign and return an Affidavit of Eligibility, a Liability Release and (where lawful) a Publicity Release, together with a completed IRS Form W-9, within fourteen (14) days of notification. If the potential winner cannot be contacted, does not respond in time, is found ineligible, or does not comply with these Official Rules, the prize may be forfeited and an alternate winner selected at random from the remaining eligible entries.

8. Publicity

Except where prohibited by law, acceptance of the prize constitutes the winner's consent to Sponsor's use of the winner's first name, last initial, city and state to announce the winner, without further compensation.

9. General Conditions

By entering, entrants agree to be bound by these Official Rules and by the decisions of Sponsor, which are final and binding. Sponsor may disqualify anyone who tampers with the entry process or violates these Official Rules. If fraud, technical failure or any other cause beyond Sponsor's control impairs the integrity of the Sweepstakes, Sponsor may cancel, modify or suspend it, and in that event may award the prize at random from eligible entries received before the action.

10. Release and Limitation of Liability

By entering, entrants release and hold harmless Sponsor and its affiliates, suppliers and advertising agencies from any liability arising out of participation in the Sweepstakes or the acceptance or use of the prize. Sponsor is not responsible for any injuries, losses or damages of any kind arising from participation in the Sweepstakes or the acceptance or use of the prize. Nothing in these Official Rules limits liability that cannot legally be limited.

11. Governing Law and Disputes

These Official Rules are governed by the laws of the State of Wyoming, without regard to its conflict-of-law rules; the Federal Arbitration Act governs the arbitration agreement below. Any dispute arising out of or relating to the Sweepstakes will be resolved by individual binding arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, except that either party may bring an individual claim in small-claims court. Hearings may take place by video or phone. ENTRANTS WAIVE ANY RIGHT TO A JURY TRIAL AND TO PARTICIPATE IN A CLASS OR COLLECTIVE ACTION. If the class-action waiver is found unenforceable for a claim, that claim will proceed in court, not in class arbitration. Any matter that is not subject to arbitration will be heard only in the state or federal courts located in Sheridan County, Wyoming.

12. Privacy

Information collected from entrants is subject to Sponsor's Privacy Policy at fascinahealth.com/policies/privacy-policy. Free-method entrants are not added to marketing emails unless they separately opt in.
